It was Signify Health’s mission to facilitate the transition from fee-for-service to value-based care, we created an electronic health record that allowed nurses, case managers, and social workers to monitor the patient population of their hospitals and follow patients who fell into a particular diagnosis related group from one setting to another.

We spent a lot of time on Episode Connect trying to understand its many users and their workflows. This was months of interviews, shadowing, ethnographic research and field trips to hospital systems across the northeast to really grasp the tools at a nurse’s disposal or the limitations in reporting and configuration in the hands of an insurer or a tech implementation team.
